We thank Dr Dauby for his comments regarding our article on routine childhood vaccination responses in children with perinatal HIV. Current recommendations about the timing of routine childhood vaccinations are based on responses in normal, healthy children. Responses to many vaccines in children with perinatal HIV infection, including the measles, mumps and rubella vaccine (MMR) and other measles-containing vaccines (MCV), are associated with underlying HIV disease status, including CD4þ cell count, viral load and whether or not the vaccinee is receiving antiretroviral therapy (ART). Because receipt of ART is associated with decreased HIV replication and improved CD4þ cell counts, immune reconstitution is likely the reason for improving immune responses seen in children receiving
ART. Persistence of protection is multifactorial and may be related to poor primary vaccine response, impaired ability to develop memory or diminished memory cells.
